Read about our boarding policies prior to having your kitty stay with us.  This form must be initialed and signed in acknowlegement prior to your cat staying with us.  For regular guests at Island Cats, this form only needs to be acknowledged and updated in your medical record once every twelve months. When you can't be present at your cat's exam, feel free to schedule a drop-off appointment.  Print and fill out this form to accompany your cat's exam so the doctor has a complete picture of how your cat's doing.

If you have made arrangements to perform your cat's blood glucose curve at home, you can download this worksheet to enter your cat's hourly bg's.  There is contact information at the bottom of the worksheet on how to return the worksheet to Island Cats prior to scheduling the doctor consultation to review your cat's results and update his/her treatment plan.
 
Have you scheduled a behavior consultation at Island Cats?  Please print and fill out this questionnaire for the doctor prior to your consultation.  Please fill it out thoroughly and feel free to write any additional notes, questions, or comments on the backside of the form should you need extra space.  There is no such thing as too much information when diagnosing and treating inappropriate behaviors in cats.  Your behavior consultation may take up to an hour so please plan accordingly.
